It is the most bizarre thing I've read recently. Which is saying something.

Desperation

It's not surprising, really, that people don't get it. Most people have never felt it, or maybe they have but not on that level. Most people who have probably wouldn't talk about it except in general terms because, let's be honest, it sounds crazy and it often is.

As far as what would drive someone unqualified to claim they could perform that surgery, called an orchiectomy, and offer to do it... That I can't say.

Lots of people do go to Thailand as you can generally buy first class airfare, hotel, meals, and your procedure for less than the base procedure costs here.

There are downsides though, recovery in a foreign country, difference in techniques, little recourse if something goes wrong down the road after you're home.

I'm sure you really want to know all of this.

I read an article about that over there. Also no waiting period, which I believe most places in the states require.

Friend's ex father in law did his/her surgery over there.
Another friend's uncle conducted the stem cell research and had his fertility clinic in Belize while being a respected doctor stateside.
Danish used go to Sweden to marry non-Danish.
Finns used to take a cruise to Russia to get s**tfaced due to alcohol prices.
I got my Private Pilot cert in the USA because at the time there was no legislature for it in Ukraine, flying close to the airport at or below 1000 feet is all you could do.

Certain things are more socially acceptable and affordable elsewhere. Beats doing it in some dark alley.

You will still need recommendation letters, most US docs still follow the WPATH so it will still be a wait to fulfill the requirements, but there will be a shorter waiting period for your scheduled date.

Surgeons in the US are over 2 years out and you have to pay cash up front, 25 to 30 grand, or get your insurance to agree to pay ahead of time.

You can imagine, then, what someone might do from a place of desperation.
