I won't say that a conclusion is arrived to off the cuff but the funnel of investigative logic tends to go toward pilot error.

The funnel of investigative logic should go where the evidence takes it. Pilot error isn’t some nebulous name, it still has to be defined if it’s cited as a definitive cause. It’s not pilot error, ok, all done here, case closed. Unless it’s a situation like I outlined above where all other evidence has been exhausted and ruled out and cited in the report as having been done so; and even then it’s given the caveat of “for unknown reasons”,

They burned a piano at Hook_Dupin’s wake six years ago.

It was one of the most beautiful and sad things I’ve ever seen. Ran across a couple of old photos with him yesterday and it still hurts.

It's a bit more complex than that. That MXS was basically divergent in pitch. Very aft CG in normal config. If that elevator got stuck and he had to unjam it, the pitching moment would be insane.

Yes, I understand. The way the report reads he leveled off and flew down the runway a bit. And then abruptly pitched up. Meaning he was in control until he wasn’t? Or was he already in trouble as he came over the numbers?
 
I’m going to guess that if Rob couldn’t recover from that failure it’s the definition of an unsurvivable event. RIP. I’m curious about the design of the plug but I’m sure the NTSB will answer any and all questions in their final.

speculation only, but it certainly doesn’t look as though there is any secondary retention method such as safety wire or pin. Who knows if the internal threads have a metal or nylon lock like a locking nut or nutplate would, or if it simply relied on torque possibly with the help of loctite to retain it. In retrospect it’s certainly the kind of place you would want a secondary retention method beyond just proper torque (which can be tricky as is with a slot head, again, in retrospect you’d probably want a torx or Allen or something).
