OK guys and girls here’s the lowdown on the PEPC forms.
[FONT="]FAA Form 8500-8, Application for Airman Medical Certificate.
do not fax this form in, you will complete an official carbon copy form at the PEPC, but fill it out before hand and then just copy all the info onto the Carbon copy form.
OF 306, Declaration for Federal Employment[/FONT] [FONT="]
DOT form 1631, Disclosure and Authorization Pertaining to Consumer Reports Pursuant to the Fair Credit Report Act- fax in both of these forms and also bring back up copies to the PEPC.
Also Fill out your E-Qip electronically and release the Document via the [/FONT]
[FONT="]E-Qip website as instructed. Bring the authorization form at the end of the E-Qip to the PEPC. This Form authorities a Background check and needs to be singed and dated.
*You must fax in your OF 306 form first in order to be granted access to the E-qip. So you should complete the 306 ASAP and fax it in because it takes about a week to get E-Qip access. I also recommend filling out the Sf85-P for Terminal and the SF-86 for en-route first and then just transfer the information to the E-qip when you receive access. Be aware that Terminal fills out the SF85-P and en-route fills out SF-86 for E-Qip[/FONT]

**be sure bring copies of all these forms to the PEPC and copies of any arrests, court ordered drug or alcohol treatments or medical issues, if you don’t you clearances will be significantly delayed. I cannot stress enough the importance of having all your documents and paper work in order. [/FONT]
