Are you picturing some crazy scenario in which someone gets a lawyer or advocate to fight tooth and nail with the FAA as soon as the words "special issuance" are mentioned and thinking that's the way it usually goes?
Sorry. Not even close to the reality,
particularly when it comes to medical issues. in most of the bad ones, the process of attempted pilot compliance and agency rejection and, sometimes fixation on denial, has gone a long way before any specialized assistance was sought. in fact, pilots with a potential medical issue would, as I think you already suggested, fare far better with advance professional aviation medical advice to get their ducks in a row before they ever fill out a medical app.
