I'm sure. But this is a good reason to ALWAYS load the approach, even if it's clear and a million.

It's 3am, I haven't slept in 20 hours and we're going into an airport with very close parallels, I'm on the localizer even though it's cavu.
But even if I'm going into an airport with no other airports for 300 or more miles and there's no approach, I'll tap the OBS button on the GPS and give myself runway alignment. It's just one more thing you can do to keep from something like this happening.

I don't disagree with you, because I do the same thing but...

Fatigue will make you do some really, really, really strange things, like not load the approach when you normally do. And maybe the guy next to you who also normally loads the approach misses it because they're dog tired, as does the third guy who is half asleep.

The runways are nearly aligned. They could've even loaded it up, saw the needles moving, and then looked outside without verifying that it was all centered up. Lots of possibilities, and we don't know any of them. ;)

Oh, I'm quite aware. I was just saying it's one more layer that can help. I don't see any other pilot I work with do things like that, but we don't exactly have pilots known for IFR abilities up here.
