The Certificate was probably reissued when the INSTRUMENT AIRPLANE rating was added.Doesn't look like it. His PPL was issued less than a month before the crash.

Kind of like experiencing your first real emergency in your career. No matter how many times you’ve practiced in the sim or on tabletop, it’s still going to be a stressor, because it’s the real thing. With real consequences.
I heard a "USAir" callsign once in the sim, years after the merger. Just skipped cactus all together.I’ve seen “that last airplane come out” when people get themselves stressed during OE.
