I swear, if this ever happens on a flight where I'm riding in the back... I'm probably going to start hucking people's bags (and possibly them) out of the way as I push pax and myself out the damn door. Idiots.

Oh yes you can def believe one of those injuries will be me kicking someone down the slide - "A Christmas story" style!

I hear a rumour that a take-off clearance was issued to a succeeding aircraft, which would imply the controller may have thought the airplane was airborne. If it was... then what happened?

Still may have been rolling. There are several airports I can think of, including KPHL, where they give takeoff clearances to aircraft holding short as another is still rolling down the runway.

I think it's a time distance thing. They, cab controllers, know for the most part how long it takes an aircraft to start rolling from the hold short line. That mixed with an aircraft hurling itself down the runway gives enough separation. Now I could be totally wrong so don't take this as law.
