I hate to point this out, but you have to go below the belt to bust the balls, in a literal sense.

Although in Mark's case, it's probably below his knees.

The bottom line is this: Do you want to work at a place that encourages reporting of safety related issues, or do you want to work at a place where people cover up their errors for fear of getting fired? Which is the safer place?

If you look at this thread, isn't that a bit of a false dichotomy? I think many want the safety and protections a "just culture" in the workplace but don't mind shooting from the hip when having a beer with friends ... or posting on an Internet forum.

Given that even in a just culture there is a threshold for termination, there will continue to be a motivation to cover up incidents. Just as nobody wants to get fired for cause, nobody wants the first of three metaphoric strikes.

A perfect system will continue to elude us.

Boris BadenovThis is an example of an Unjust Culture. This was not a made up situation. Firing the Captain without fixing the problem only created a more hazardous environment, which is why this industry is trending toward Just Culture in terms of safety. seagull said it better than I: It's not about punishment, it's about preventing future accidents.

There's an obvious disconnect here. I agree with everything you wrote. I think the Captain in question made a good decision and should be lauded, not punished. Where I start to lose you is where this applies to a guy who crashed a perfectly functioning airplane on a perfect weather day. I'm not opposed to "Just Culture", nor have I ever said that I was. I'm opposed to applying "Just Culture" principles to guys who screw up big in a way that is, well, "negligent", if I may trot that chestnut back out, and (even more so) the bizzare cultural orthodoxy that seems to arise around such situations. Crashing an airplane and killing people because you can't fly a visual approach is not a "retraining" event, IMHO. I'm not at all clear on how the two scenarios seem similar in any way to anyone.

Visuals are more fun in a metro.

Climbing out is better in the bro.

Below 10000 is more fun (and faster) in a metro.

Getting up to FL280 and moving at a good pace is definitely an advantage of the bro.
