He is by no means in a minority. I'm personally aware of two other companies where this happened; I'm sure there are many more. The FAA has yet to begin to scratch the surface of a proactive fatigue policy, even after 3407.
It happens more often than you may think. We had a guy blow through a bravo, luckily he had a working AP, or it might of ended differently.
