You go to the nearest ETOPS airport. Today with the weather we are using CYFB(Iqaluit), ENSB(Svalbard/Longyear) and UIBB(Bratsk).
It is also what they did in this situation as they had not gone past the critical point to keep going to KOA.

I think between ditching at 87n or just augering in, I'd choose the latter. Nobody is coming to save you up there, at least not fast enough
