It was the 3rd commercial use 777 ever produced in 1994. Of course the media is saying it was an old ass airplane, and they're kinda right but I doubt the engine is that old.
 
Just out of curiosity, how could a cowling come off? Someone just forget to make sure it was secured after the last engine check or something?

In several similar instances it has been a fan blade coming apart that ripped the inlet and cowling off. There was a picture either earlier in the thread or on another site that showed the engine head-on and a very prominently missing fan blade.
