There was a thread on JC some time ago about CFI's being held responsible for flight review signoffs for up to 12 months. I can't seem to find it anywhere. Can somebody point me in the right direction please?

FAR 61.189 requires flight instructors to maintain records of endorsements for a period of at least 3 years. Is that what you are asking? Sorry if I misunderstood.
 
I don't think there was actually a rule passed, more like a discussion about it. Maybe it was on another forum.

There was one remark here that for any airman involved in some mishap, the FAA was going to send letters to the flight review instructor informing them of the event. Nothing punitive, just an informational letter, word to the wise sort of thing.
