Questions on form 8500-7

Last month, I had my annual eye exam and had the Dr fill out a form 8500-7 for me to take in when I go to renew my medical certificate later this month. How long is the form good for? And does this take the place of the eye exam at the AME? In the past, even though my eye doctor said I was corrected to 20/20 in each eye, I'd go to renew my medical and couldn't see better than 20/25 on the AME's eye chart and could only get a 3rd class instead of a 1st or 2nd class. I can only conclude that the AME's equipment is faulty. If he won't accept the 8500-7, should I find a new AME?
 
If he will not accept the 8500-7, find a new AME or a new AME with better equipment. There are some tests the eye doctor does not routinely perform such as the tests for Phorias. We do thiose if they have not been done by the eye doctor.
 
Is there a time limit that the AME should accept an 8500-7? I get a first class every six months, but see my eye doctor less often then that.
 
Update: I went in for my medical exam this morning, handed over the form 8500-7, and the Doc accepted it... sort of. He didn't understand why I brought it in. I'd had LASIK surgery back in 1999 and he first thought I brought it in for that. "No", I said, "I got new eyeglasses so I could pass the 2nd Class eye exam." "Oh", he says, "This form is only if you had LASIK or some other eye surgery You didn't need to bring it in" So I had to do the vision test anyway but I passed it with the new glasses, and got my new 2nd Class Medical certificate.

On the one hand, I did pass the eye exam- and should be expected to be able to pass it, but on the other hand, I thought the 8500-7 was supposed to eliminate the extra eye test
 
The 8500-7 is a report of the eye specialist.
My question now, though, is does that report- for instance, if the report from the eye specialist says my distant vision is 20/20 or better in each eye- does that mean I shouldn't need to repeat the distant vision test for the AME?
