Okay so people have been calling and getting some info at the number listed on the asap...

...do you think it's possible that they might respond to the email address listed? My hubby isn't really able to call during business hours and we really want an answer.

I think it's worth a shot...might not be as fast as getting an answer over the phone but maybe you'll at least get some kind of answer...Go for it!

I sent the forms in a couple days ago. Different person for E QIP. Just finished up my equip and faxed in the attached form as well. If you havent already I would start filling out a blank sf86. It can be rather time consuming. I did it for the military so all I needed was to update it and it still took a couple hours to fill in the online portion

Besides the SF-86 and Fair Credit form, what else was included? I just want to prepare myself for paperwork time...

ya I know. Was sarcasm.... The sf86 is the only thing that they wont send you but is the same as the e-qip but you can use a blank one to start gathering info. Make sure you go back 10yrs just like a military TS app.
 
Maybe I am unclear of the paper work. But just to be clear on what everyone else is getting.

I got from my HR rep
-3300 a record of family relationship
-and an applicant contact info. sheet

I filled out both of these and first faxed them to her.
She said she did not receive them yet so I scanned them and emailed in pdf format.

From my PEPC person (not sure what to call them but the email address that sent out PEPC info) I only got....
-8500 applicant for airmen medical
-and an of306 declaration for fed. employment.

From what I understood we are supposed to fill out the 306 form and fax that in. The other sheet we take with us to PEPC

Is this all correct. Reading the posts it seems like Some people got more in their emails to fill out. Maybe this is different bc I am terminal???
 
Sounds like what I received. You will then receive another e-mail once you are authorized for E-QIP for your security clearance.

That's correct. When you get the email that says you were authorized for e-qip, there will be one more document attached to that which is a DOT fair credit form. e-qip is all electronic... hence the "e."
