Your field investigators, yes. Your board itself still consists of political appointees, and the agendas that can come with that. There have been good boards and members, and not so good ones. NTSB is always one accident away from being the next CASB.

I wouldn't say there's "seldom a singular event", I would say there's "never a singular event"; which is why the "accident chain" model is kind of incomplete, whereas Reason's Swiss Cheese model is far more thorough.

Yeah, I left it open as "seldom" because I figured there'd be someone on here that decided to geek out and research some obscure incident that was determined to be caused by a singular event and wave it in my face.

But yes, again I agree with you.....Swiss Cheese model is a much better analogy to aviation incidents/ accidents.

Fully agree partner. I think the two can be inter-related too. For example, individual parts of the Swiss Cheese model having "event chains" of their own; which at the proper time, allow that hole to be exposed that the accident potential passes through for that particular "slice" of the cheese. If that makes any sense at all (my own description of that sounds convoluted to even me) :)
