Narrative:
A Piper PA-46-350P Malibu Mirage, N97CX, and a Cessna 172N Skyhawk, N160RA, were destroyed when they collided at North Las Vegas Airport (VGT/KVGT), Las Vegas, Nevada.
There were no survivors.

Preliminary information indicates the Malibu was instructed to land on runway 30L but approached 30R, the Skyhawk was on approach to 30R and a collision occurred.

But like they say, What happens in Vegas stays in Vegas... until Lake Mead dries up and tubers vid it in the mud flats.
 
I can see more emphasis being put on parallel runway ops soon. FedEx landed on the wrong parallel runway in Tulsa last week or the week before and United did it in PIT last month. Not to mention the other times that folks have lined up with the wrong runway and trapped that error.

Several compatriots directly witnessed F9 getting waved off from 9L in ATL with a Turkish 777 lined up for takeoff in broad daylight last Friday. 9L is rarely or never used for arrivals. F9 was over the approach lights.

A SWA was cleared for takeoff on LGA 13 when I was inside 1.5 miles for Rwy 22 just yesterday. I was getting ready to put my go-around pants on, but the Southwest guy said “uh, with Delta on final?”

The threat is absolutely out there. We have things like TCAS, 2 pilots flight decks (although F9 must have had all their eyes closed), and standard phraseology and procedures to trap the errors. These people didn’t have most of those tools. Stay safe out there.

Yeah we have the same, and I'm sure most other operators do as well. I've definitely caught myself saying "Runway XX verified" without, you know, actually verifying that it IS Runway XX, but only when totally loaded up with other surprise tasks. It's not perfect, but it's a pretty good policy, imho.

No. You're lying. The only time anyone has said someones eyes were glowing was after the fact and they never said it to that persons face. I have big, beautiful blue eyes that will look directly at you and make your knees weak, but no one has ever said they glowed. Pants on fire you kook.

they do opposite direction ops at LGA? Or did they just do a runway change?

But I’ll clear people for 22L and them reading back 22R is not an uncommon (multiple times a day) occurrence.
