May 2008 CLT Runway Incursion Update

She lost SA on that one. Although, 9DR should have had the sense to at least look down the runway before taxing into position - hell - if only they had been listening they would have realized someone else was coming their way.

Controller's fault though.

Yup. The swiss cheese holes almost lined up on this one. The controller messed up, the jet crew was concentrating on the T/O roll (as they should have been), and the Pilatus crew didn't look/listen. ADE-X for the win.
 
I know the crew. They did a damn good job preventing what could have been a disaster.

The PC12 pilot didn't listen to the bigger picture and didn't check up the runway (although slightly in his defense, due to the hump where 23 crosses the runway you can't see the top end of 18L.

The controller obviously didn't check to see WHERE the PC12 was going into position and hold and for what ever reason assumed that he was at the top end of the runway behind the CRJ.

ASDE-X did catch the situation but unfortunately the controller gave terrible instructions once she saw the alert and it was all but useless in this situation.
 
I think the controller's just get into this rhythm of things, Clear one for take off, issue the next position and hold, exit the aircraft from 23 to ground, switch the takeoff to departure, clear one to land on 23, clear one for take off, issue the next position and hold...

And she was just in the rhythm and issued the next position and hold, not thinking about the fact that it was downfield from the departing jet.

Scary. Glad it wasn't two larger planes that couldn't occupy the same runway at the same time...
 
Very scary for all involved. How would you have liked to be seated as a passenger on the left side of the CRJ? Yikes.
 
I know I am probably opening a can of worms on this, but where was the media firestorm over this and the release of this from the FAA?

Two pilots overshoot a destination and are put up against the firing squad. This controller nearly collided two planes. Where is the media calling for nationwide overhaul of the ATC and airport operations? Was the controllers face plastered over the news?

Sorry for the rant and if I am out of line I apologize.

All 3 parties involved have their part to blame....9DR should have had better SA, 390 should have caught on when TWR cleared the Pilatus onto the same rwy...though she didn't mention they were an intersection departure. I totally agree the TWR controller was useless once the alarm went off.

Hey guys I apologize if this is a stupid question but what happens after an event like this. It sounded as if she was sending the crew back to 18 (Holding short of 23) to take off again. Would you not cancel the flight and an investigation ensue? Or would you just collect yourself and go ahead and depart?
 
Scary indeed. I thought the proper phraseology was something like "N409DR, RW18L, taxi into position and hold via intersection A". Isn't that how it's done elsewhere?
