Lost License/Medical

I'm not gonna go as far as saying it's lost yet, but I can't find my wallet (containing license and medical) and i have an early flight tomorrow morning. I know there is a form that can be filled out via the FAA website, which I can print out and use as my license/medical if I cant find the wallet, however I cant find the webpage.

You have to call a number with the FAA and they can fax you a temporary authorization to use a medical, its good for 60 days. I know you can get a temporary authorization to excercise airman certificate online but I dont think you can get the medical without calling the dupe desk.
 
405-954-7674 Leave a message with your name, SS, phone #,and fax number. The person will call you back to reauthorize it and for you to verify it before they send a fax. It's good for 60 days.
 
You may request temporary authority to exercise certificate privileges of a valid airman and/or medical certificate or verification of an expired flight instructor certificate in the form of a facsimile (FAX) or e-mail.

1. Go here: https://amsrvs.registry.faa.gov/amsrvs/logon.asp

2. Create an account (or log in if you have one)

3. Click on "Request Temporary Authority to Exercise Certificate Privileges."
 
You can request a dupilcate copy of your medical so you dont have to take a new exam if you dont want to.
To obtain a duplicate medical certificate, fill out an Application for Replacement of Lost or Destroyed Airman Certificate(s) and Written Test Results http://www.faa.gov/licenses_certificates/airmen_certification/certificate_replacement/ and complete the section for replacing a medical certificate. You’ll need to include a check for $2.00 and mail it to:
Federal Aviation Administration
Airmen Certification Branch, AFS-760
P.O. Box 25082
Oklahoma City, OK 73125-0082

from this same website you can also request temporary authority to exercise certificate privilege
 
Well I went on the FAA airmens services site and found the one for my pilots certificate but i cant find the one to replace my medical.. my medical went through the washer and dryer so if i can get a new one i could throw this winkle thing away!

Yes...Turned the wallet turned up under a dirty pile (and I mean pile) of clothes in my room. Maybe it's time to keep more tidy. Difficult waking up one day at 4 am to fly, and the next day showing up at 10 pm for a flight.

Jbroz- If you follow the link clocks posted, you can print out a temp (120 day) authorization for your cert, and medical.
