I was diagnosed with Chronic Myelogenous Leukemia at age 19 and have been on a medication called Gleevec ever since. I am now 25 years old, and my condition has been in remission for 6 years. I have been seeing a hematologist every 3 months for the last six years, and as you can probably imagine, I have a medical file about 4 inches thick full of lab results, etc.
I am applying for an ATC position and am wondering if I need to bring documentation other than my declaration on form 8500-8. If so, how much documentation should I bring with me?
