Just a question about I,II,and III class medicals

I have a question: If for example you fail to pass a first, second, or third class medical, does your failing to pass it go on some kind of record that could come back to haunt you in the future per say?

A follow on - if I'm 30 years old, with no medical issues, what exactly is different between the three medicals? As best as I can tell, vision needs to be corrected to 20/20 near instead of 20/40. What else is different?

(Probably should have done the Class I last year, since I'm going to need a Class II in a few months....)
