How something like this could happen.
A pilots worse enemy during an accident analysis is another pilot, so I am not gonna cast any judgement here.... but I am really really close to do it...
I don't have any details of the flight other than this video.

From what I'm hearing the pilot never had any prior training of any sorts
So with that said, that wasn't the worst possible outcome
