Twice, but neither ended up in us having to depressurize the airplane.

Interesting, my roommate was on an America West flight from Las Vegas to Oakland when the windshield shattered 15 minutes after takeoff about 3-4 weeks ago. However, I don't think it was as bad, they just turned around.
