It has in the past. There was on cirrus that a passenger pulled the chute on when s/he saw fuel coming out of the wing from a fuel cap.

I wouldn't fill out a report... is it illegal to fly without a gas cap???? (I wouldn't recommend it, but I don't think you need to fill out a ASRS)

ASRS reports aren't just about CYA on potentially illegal actions...NASA is interested in anything impacting safety. It's always worth a report to help your fellow aviators.

Report to NASA enroute. I hope they dont think this is too trivial. If nothing else, filling out the form made me think about the factors that led to me forgetting to replace and secure the cap.

Glad to hear you've submitted the report! That's the other benefit...not only does it help others who read your report, but just filling out the report also makes you think a little more about things that led to the event and how you can avoid it in the future. Makes you a better pilot in the end!
