Be very careful about this at Pinnacle. The ERC member from the FAA at Pinnacle has stated that fatigue issues are not admissible into the ASAP program. He says that submitting one after flying fatigued doesn't qualify because you intentionally flew when you knew you were unsafe. Intentional actions are not covered by ASAP. If you you refuse to fly and get disciplined for it, it's not admissible into ASAP because ASAP only covers events that take place while flying, so refusing to fly is a non-reportable event. The only way to do it where you might get it accepted is by reporting that you felt fine when the flight started, but you realized you were very fatigued towards the end of the flight.

I wrote my master's thesis on aviation fatigue. It's possible to function on little sleep and feel fine, however just because you feel fine doesn't mean you ARE fine. Accident rates go up rather dramatically with fatigued pilots. On a similar human factors topic, there was study (1999?) that showed fatigued police officers could shoot accurately when fatigued, but their vcircuit breakers for decion making were failing them. I can't see us pilots being any different...
