Er...ATP Signoff!?

Is that not a line check, and not a check ride? 121 is chineese to me.

If you are getting a full type rating it is a normal type rating ride either done by the FAA or DPE and 8710's are filled out.

then there are

PC Proficency checks (capts take them once year). It can be failed and is basically a check ride done in the sim and admin'd by a Check Airman.

PT Proficency training (capts take them once a year). Same stuff as in a PC but is normally non-jeopardy unless you really screw the pooch and they also throw in more stuff you seldom see. aka. smoke in the cokpit, single engine approach in the 727, standby power approach, etc.

the above checks offset by six months so the capts are in the sim every 6 months doing a PC or a PT. The FO's alternate yearly.

Line Checks is usually done by a check airman and they just ride in the jumpseat and observe and then point out anything non-standard you are doing and add suggestions on technique or other minor items as necessary. Then you have the ones done by the FAA which are really no big deal unless you screw the pooch on something then it is time to talk with MR FAA depending on the item. Most of the FAA guys that I have had in my jumpseat are real world and down to earth. I have only had a couple that were a PITA.
 
You used to have to get a sign off for the written by taking your logbooks to the feds and having them paw through them. At least that was my memory, and not a particularly good one.
You are correct. I got my ATP in 1998 and prior to taking the written you had to present your logbook to the FSDO to get the documentation to take the written. I dont know whether that has changed.
