BA 38 lost both engines due to heat exchanger issues and ice crystals. And are you really using US Air 1549 as an example of a dual engine loss?

Well, how about the Airbus A400M that had 3/4 engines at once go to idle thrust because of a software bug and crashed?

Software wipe mx error prior to the delivery flight.

Ok you guys are right. It’s totally common and plausible to have a part 25 airliner have the engines shut down prior to takeoff. I hate when that happens!!

A friend of mine was a Captain on RR powered 787's. He says there is a Tech Bulletin on this very issue.

He was recently moved to GE powered 777's and is very happy on the 777 despite the older technology.
 
Looks like the current target is software that shuts an engine down when a reverser deploys in flight...

And ANA isn't without its own bone headed incidents:

Japan's Transport Safety Board said the captain had stepped briefly out of the cockpit, but when he returned the co-pilot inadvertently operated the rudder trim control while attempting to let the captain back in.

The 737 banked slightly right, then twice rolled sharply to the left, achieving a bank angle of nearly 132° and "rapidly" pitching 35° nose-down, the inquiry board added.

It descended at high speed, losing 6,300ft before recovering at 34,700ft and climbing.

hah.....fair enough. I've seen sympathetic failures in my jet (they are inches from each other), but I know there have been some similar transport category failures along those lines too historically. Touche

It’s worth noting that a bulletin was released by Boeing not so long ago to pilots and maintenance crew about the Thrust Control Malfunction Accommodation (TCMA) system, which prevents risk in an uncommanded high-thrust situation, stating that errors in the landing sequence could cause the system to activate. The errors include a combination of selecting full reverse too quickly before the aircraft has transitioned to ground mode followed by a quick deactivation of reverse thrust. Boeing advises not to apply full reverse too quickly in the bulletin.

That’s a bit of an overreach, I don’t recall laughing about that on a public forum. I’ve got nothing against offensive humor, but I’d give my buddy a discreet elbow in the ribs if he was loudly doing a bad accent while making fun of a different ethnic group in a public setting. (Or even if he’s making fun of his own ethnic group, despite not looking like he belongs to said group, ahem, you know who you are!)
But that’s just me.
