That was slightly tongue in cheek. The single point of failure on the Brasilia propeller control system had an "extremely remote" probability of failure, and failed at least twice resulting in fatal accidents (and several more times during the feathering check on the taxi-out).

10e-6 is not "zero."

Gotcha......sounds like any number of other aviation systems. That low probability of failure component is never as reliable as the abicus counters hope
