Color Vision Test - Falant or Optec-900 near Chicago?

Does anybody know of an AME or optometrist within 100 miles of Chicago that has a Falant or Optec-900 test? I need to find one to try and get a flight restriction lifted off my medical. I read of one in Columbus, but if anyone knows of one, please reply to this post.
