Appendicitis surgery and 3rd class medical

I am going back to flight training to get my private this summer and I wanted to renew my 3rd class medical and I wanted to know about a surgery I had 4 months ago for Appendicitis and the info that the AME and FAA need for my medical. Do they need any medical records and any other info that they need from the doctors and hospital and what would I have to put down on the application and present to the AME when I go?

I had one in '06. Renewed my medical last year and had no problem. I just mentioned it on the appropriate line.

Had mine out about 10 years ago with Severe complications-Dr's fault-long story short I hold a 1st class. Just let the AME know.
