Acid Reflux

Hello Dr

Went to my family doc this morning about heartburn and acid reflux that has been bothering me for a few weeks. Among other tests he did an EKG (normal) and then prescribed dexilant. I checked and that is on the approved list.

What do I need to put on my next medical application?
