Accident/incident report

I went to the page you suggested but could not find the info I needed. I am looking for info on how to obtain my accident/incident report. Even though it is clean I need it for an application so how would I go about receiving this information. Unless it is on that page you suggested and I am just a little slow.

If you're looking for something that you were specifically involved in and if it's on your record, go:

main page > pilots toolbox > airman record request

if it's on your record, it should pop up there but you've got to fill the form out. It's the FAA after all!

This one is the FAA's database site. It has more powerful search features than the NTSB site, but it accesses the same database. Also has the "FAA Incident Database," ASRS/NASA reports, and some other goodies.
 
Well through some research and then calling around to a few places I received a phone number to call.

The number is 405-954-4173 which is for the FAA and the certificate office of some sort. I then asked her the same question and she stated that I need to Fax a paper that included that I wanted to receive my Accident/Incident enforcement report and to also include.

Name
Date of Birth
Certificate type and number
Social Security Number
Address where you want the report to be sent.
And make sure that you sign the letter
Then fax this information to 1-405-954-4655

Hope this information will help someone else in the future, and Doug you may want to post this info under tools. Thanks for you info. David
