Accident Case Study

Excellent video put out by Aviation Safety Institute. It’s the Learjet crash at TEB from a couple years ago. I know it’s been reviewed in depth on here before but it still baffles my mind any time I hear about it.

There's a whole series of them on Youtube and they're all great. This particular one was pretty horrifying though.
 
That whole thing was such an unbelievable cluster that it's hard to wrap one's head around. I've worked for 135s which ranged from "sorta scary" to "surprisingly good", but even the worst of them were orders of magnitude better than that poop show. I flew the same trip regularly when I worked for @CK, and while I'm sure no one would accuse me of being Bob Hoover's illegitimate son, even for me, it's genuinely difficult to figure how that many holes in the swiss cheese aligned on the same day. You think you're "hundreds" of miles out when you're really 40-something and you STARTED at 80? You can't be bothered to listen to the ATIS, or at least let the other guy read it off for you? I mean I'll stop, because the list obviously goes on, and it's not like I've never intercoursed the canine, but ffs. I feel bad for the "kid" in the right seat (although did I hear correctly that he was 33?), he seemed in over his head but with the right instincts. If there's a lesson here, it has to do with what, if any, oversight the FSDO was providing for this company. No WAY this just happened out of the clear blue.

A propos to nothing, though, I swear I get PTSD every time I see "VINGS" or "DANDY" depicted on anything, anywhere. Just say those words and I'll probably dive under a table.

Now imagine how many of these types have slipped though the radar and are at 121 pax carriers. Scary thought. But facts show both the two most recent big 121 crashes (Atlas and Colgan) had questionable pilots that passed through, both having lied about themselves too.

Yes, and both, AFAIR, came to their new jobs from other 121 carriers. Quite a number of them in the case of the (Atlas) F/O! Huh. Also, what is meant by "too", here? I see no indication that either of these guys misrepresented their history or experience.
