Oopsie!

Flight plan, logbook, placard. The trifecta!
On the Brasilia, it was:
Code:
AIRCRAFT TAIL NUMBER ... (___) CP/FO
where he read it from paper and metal, and I read it from the can.

On the CRJ, it was corrupted to:
Code:
AIRCRAFT TAIL NUMBER ... CHECKED CP

and on this thing, we don't have it at all.

We had a few (CRJ...) operations blast off without the maintenance can, and IMO, just changing the checklist dialogue to that of the EM2 would have solved the problem. Instead, we enter an ACARS code with the page number of the airworthiness release from the logbook. (groan)
 
From a cold dark airplane, we have 3 times we check that either the AML is on board or we are checking the maintenance status of the airplane via the AML.

Then we have a single checklist item on the before start checklist.

Of course, it is sort of hard to miss the monster of an AML we currently have.
 
Don't forget the big ass red ETOPS sticker on the front cover of the logbook. Very hard to miss that if you actually pick up the logbook to check MCOs. Who trusts dispatch and MCC to get those right without reconciling the paperwork with the book?

I can remember writing 3 things up and calling MX at the request of the captain. A few days later I get the "you guys were flying with open write ups, fill out an ASAP, ASAP!" So I did. When it was closed, I got the "be more careful checking the can, you guys got lucky" email response.

"Did'ja even bother to read my ASAP!? How is the guy who wrote it up, and called MX with the log page numbers, supposed to verify that MX did their job and fixed the thing #1, and #2, why didn't the crews who flew it after us, write the same stuff up?"

Put it on the checklist! :)
We did! It is the first item in our Before Start boxed items checklist. Since I am responsible for the can I have to hold it up and we cross check the tail number on the can versus the tail on our paperwork and what is on the airplane.
 
If they were in the wrong tail, how did they get valid W & B numbers? Seems like the system should have caught the release not matching the Tail during the ACARS request for W & B.

Its not a real ETOPS, which is why I used the quotes. But we do operate three 340s which have an over water kit. We take out two seats and put three rafts in its place. There is also a clip on the wing we can run a rope out to in the event of an evacuation.

There are also sills that I think get installed below the overwing exits, IIRC. The majority of Saabs I flew couldn't be flown overwater and it was made clear in the manual and QRH that

DITCHING PROHIBITED on that aircraft.
