Pilot, reporter, and 1 on the ground. Covering another fatal bus crash at the same time on live TV. Looked like no chance of an auto for whatever reason.
 
Oof. That sounds like the low rotor RPM annunciator horn going off and the engine noise winding down. From a hover position that’s gotta be a tough one to handle.

That’s an AS350B2 version, which can be told by the warning horn style that can be heard. The first thing heard is an increase in Nr, leading to the high Nr warning horn that briefly sounds at 10 seconds. That occurs at 410 RPM in those birds (normal airborne Nr range is 375-405). The next thing heard is rapid Nr drop off and a low rotor horn at 15 seconds, which the pilot likely catches momentarily by collective reduction. The helo, however, is in a high hover and begins settling quick as the pilot catches the low Nr, but is unable to build any notable Nr back up (difficult to do with no kinetic energy to cash in), and as he attempts to slow the rapid settling descent rate with a raise of the collective, the low Nr horn sounds again and stays on. At this point, there is nothing available to trade in for Nr increase, and the helo is falling from the sky to ground impact.

It’s the “what”, that I can see and hear from the video. Unknown as to why specifically, but an Nr loss like that is tied to a power loss of some kind, either 1. loss or partial loss of engine power, or 2. some form of transmission problem occurring where the engine power is prevented from being able to provide power to the rotor system. What occurred specifically to set off the high Nr horn momentarily, was either something happening in that instant that the pilot could not immediately control, or the pilot’s initial reaction with collective reduction to something that had occurred in that moment, that he saw.
 
Generally speaking, a high OGE (out of ground effect) hover isn’t the place you want to be hanging out, as it provides few options, and requires rapid and proper reaction to any power loss or power train loss, in order to carry out a successful emergency landing; which in a case like this comes down to survival, as the helo is going to get damaged to some extent. But, sometimes a high hover is needed, and is a risk that is accepted. Ops like hoist rescue, rope operations, sling loading, etc, all require lots of OGE hovering, but that comes with the territory. As a news helo, especially with space-stabilized camera systems and such, I would prefer being in a 30-40 knot orbit, versus a high hover. Same thing I do using my own camera/FLIR systems on my birds.

That said, the B2 series is an older, but very reliable, version of the AStar series. Unless modified, the standard B2 has an extremely reliable Saffran Arriel 1D1 engine, and a single hydraulic system, making it a comparatively light helo. If I were the IIC, I would be very interested to have the maintenance records on the bird compiled together, to see what it’s latest Mx has been, as the field investigation begins,

So in case like this, what’s a proper rapid action? There’s zero forward speed, so what can be done when the engine fails ?

At the altitude they were at, not much can be done. It’s kind of a dead man’s zone. You can try to ease the bird forward to get some forward speed, which will allow for a flare and cushion to some form of a touchdown. But in a power loss situation, everything you do to attempt to maintain useable Nr, comes with a cost that works against you. Be it a higher descent rate, a loss of speed, a further loss of Nr, etc. You need to either have kinetic energy (airspeed) or potential energy (altitude), to give you some chips to cash in, so to speak, as you attempt to preserve Nr with no engine power to maintain it.

Helicopters have what is known as a Height-Velocity, or H-V diagram. It details what combinations of speed and altitude, an autorotation in the event of power failure, is successful; thus, it defines the “dead man zone”, where a successful auto isn’t possible. In the case of the Astar series of helos, at 0 knots, or a hover, you have to be below 8’ AGL, or above 500’ AGL, in order to have the best chance at a successful autorotation. The Astar series are a low-inertia rotor system, meaning they don’t preserve Nr rotor speed well during a power loss. As opposed to a UH-1 Huey series, which is a very high inertia rotor system, that preserves Nr exceptionally well in the event of power loss, and thus has a different H-V diagram from the Astar. Essentially, in an Astar series, in the event of power loss, where you are going to be able to autorotate to, is somewhere that’s in your chin bubble window, or somewhere that’s nearly under you and not too far forward, and you have one chance at a good landing. In a Huey, in the event of power loss, where you can land is somewhere out the front windscreen, ie- distant, comparatively. And at the bottom end of the auto, you can touch down smoothly, pick back up airborne again to a few feet AGL with no engine power, move 40 yards or so laterally, turn 180 degrees, and touchdown smoothly again, all with the remaining Nr you were left with after the power loss, it preserves Nr so well.

In a high OGE hover situation, you’re already losing Nr due to the power loss. If you rapidly lower the collective to prevent the Nr loss, the cost for that is now incurring a very high descent rate. If you try to give forward cyclic to gain some speed, you unload the rotor system……which one of the effects of unloading a rotor system, is Nr loss, loss that can be normally checked with engine power; but if there’s no power or there’s no power getting to the rotor system, a power loss that causes Nr loss, will be highly exacerbated by the application of forward cyclic, resulting in an even more rapid loss of Nr, since there’s no power getting to the rotor system to keep the loss in check. You basically have very little, to no choice in putting the aircraft down anywhere other than directly below you.

BL is, from where they were, the options were few, even if an instant reaction had been accomplished to whatever was the cause of the Nr loss. It’s why I’m never fully comfortable performing a high hover like this; even though for what I might be doing at the given moment, might require it.

From scene video, it appears the Astar came down on top of a shipping container that was on the ground in a shipping dock area, crushed part of the container and rolled to the right off of the container and to the ground next to the container, where it ignited in flames. Had the containers not been there, it’s entirely possible the flat impact on the ground would’ve spread the skids, and possibly not caused the post-crash fire. B2 models, as stock, have a non-crashworthy fuel tank located under the main rotor transmission and behind the rear seats; however, many also have conversions to an aftermarket crash-resistant fuel cell.

Cannot believe this. I listened to Eliana (the NBC reporter on board) all the time. She was so good at her job and very well known throughout LA.

I hope this is a wake up call for other media operators as they should never get complacent and put themselves in situations where the aircraft is not recoverable. They probably would have at least had a chance had they been in a low speed orbit rather than a near hover at ≈900' AGL as it appears.

Do we really need aerial news coverage of a bus crash? Why not use a drone? I think this should cause media operators to question whether the risks associated with aerial news coverage are justified. Using helicopters in this capacity isn't common in Europe. Seems to mostly be a US phenomenon?

I'd argue that it's even more granular and is an LA issue. LA seems to love its car chases and the helicopter footage of them.

Drones and cameras are taking over a good amount of ENG, or Electronic News Gathering, work in various places; it does seem to depend on the particular market, at least for now. Very large markets like the greater Los Angeles area, seem to feel like they need a helicopter to get to news events faster. That may change, possibly accelerated by this accident. ENG may very well go the way of jobs like Traffic Watch, a job that used to rely on manned aircraft, and settled for freeway cameras today. Even in news markets like PHX, there used to be 5 news helos; any station that wanted to be considered somebody, had to have one and the pilot/reporter face to go with it, or you were a nobody. After the 2007 news helo fatal midair, I think the market is down to 1 or 2, with at least three news stations sharing the same news helo. The ENG helo is kind of a slowly dying breed in this day and age, just as traffic watch went the way of the dinosaur mostly.

I had the 2007 midair in mind. I understand uses like police/military/rescue/firefighting, maybe even corporate, but for news coverage and tourism, is the risk acceptable? I'm reminded of the helicopter that lost its rotor last year after impacting birds over the Hudson and the 2009 NYC helicopter crash. There have been multiple fatal crashes in Hawaii. The conventional wisdom is that flying is the safest way to travel. Is there any data comparing the rate of civilian helicopter accident fatalities to regular vehicle accident deaths?

I’m not usually incredibly squeamish but the audio from the 2007 News helo midair in Phoenix is something I’ve heard exactly one time and never want to hear again. One of the few times I’ve felt physically ill after hearing something like that.

The video of yesterday's helicopter crash is chilling. It's pretty rare to have a crash video from the crew's perspective. I wish I could unsee that.

Yeah, in the age of so much being recorded/broadcast/etc even though it exists doesn't mean we all need to see it. Imagination of what happened is more than enough for me.
