Near miss at Midway

I put a small "Post It" with my flight number next to the master caution light.

Changing flight numbers every leg kicked my ass for a bit and the habit just stayed with me.
Even with my trip sheet in view and crossing out each leg as we completed it, that last leg was pretty tough. (I would run out of paper.)

Now, and I don't know why we don't teach this, but now, my scratchpad on the right chart holder gets this format:

I don't.

Even when they're barking "radio check", "over" and "I need your hold short of 16L!!" when we're landing 17.
"Please don't talk to me at 100 knots and we will hold short runway 16 left, SouthernJets twelve twenty four."

That's exactly why I hate the way ORD gives out taxi instructions en masse with no chance to respond. I get that they're busy and it's for efficiency, but that's like us reaching the end of the runway and going "well...we don't have time to run the before takeoff checklist, let's just go and hope for the best."

Just because you're busy, doesn't mean you don't have to do everything like normal. In fact when I get hurried that's usually hen I realize I need to step back and take a look at the overall picture.
 
The recording was interesting- I listened to it several times, and it really does reinforce how similar "southwest" and "delta" can sound when said at a hurried pace by the chicago controllers. I cannot count how many times I've had to ask for clarification up there, and it is even worse with similar flight numbers like this.

The fact that it was coupled with 31C, since that is the usual departing runway out of there increased the threat more- they heard what is a very normal call and off they went. Absolutely excellent reaction by the controller!

Someone mentioned how you hear the pushback and such after the incident on the youtoobz- that is just the way they spliced it. The push and stuff went on before. Southwest ended up taking off hours later for whatever reason, while the DL flight called the kremlin (probably texted their spouses too to pick up extra padded kneepads for what was coming), and off they went.
 
I like how Delta gets a pass on APC. If it was Southwest who had done that and taken Delta's clearance, holy cow the gloves would be off. :D

Yeah, I mean, come on. Look within yourself, talk to the Force, Luke. This is so true it practically jumps off the page and becomes a meme of its own.
