I'd bet it has something to do with international ATC and common miscommunications/misunderstanding of standard phraseology...Whats the story behind this?

As Roger, Roger points out about "Roger", it means something not at all similar to "Affirmative", yet I hear guys who really should know better using it that way all the time. Roger?
