Does anyone know the name of the company that develops the stencils and forms for specific aircraft types? I am looking to order a set with triplicate forms for a King Air B200.

Man I'm sorry this isn't the answer you are looking for, but call an FBO at a big airport like Teterborough or Van Nuys and just ask them to look at the top of a netjets form that has been left behind or something.

I used to know the name but I'm totally blanking out right now.

I would strongly discourage that. First it violates privacy/confidentiality. Second, the forms are generated from proprietary company programs and are not publicly available.
