some help on driving and FAA records..?

I'm trying to get all the info together to submit my application for the PACE program.. Can anyone tell me how to get a 5 year driving record that includes different states? When I tried this online, I had to enter a drivers license number for the state I was requesting my records from (I don't have driver license numbers from previous licenses). Has anyone else done this?

Also, can I just request FAA accident/incident data from the FAA website? How current does this information need to be for submission with my application, as I'd like to get this info now for a January interview?

I spoke with someone from Mesa today, but to be honest, they were not very helpful, and could not answer any of these questions for me.

I'm not sure about the driving record, you may just have to go to each state individually or get just the one from your current state.

I do know about the FAA accident/incident report....It took them about six weeks to get me mine, and that was after calling and sending three faxes. Those folks are seriously understaffed and overworked.

I had the same problem when I called MAPD...just not a lot of information forthcoming.

If you need the fax/info number for the FAA let me know.

I had to get my driver's record from several different states, because I've moved like 20 times in the last 10 years in 5 different states and a foreign country. You need to talk to each state's DMV, which may or may not be easy. If you're able to talk to a person see if they can find your record by name, address on that driver's license. I was lucky because I was only licensed in two states and I managed to keep a photocopy of my old one and had the driver's license number.

For the FAA Report, fax a request to (405)954-4655 with the following info:
Certificate #
Full name
DOB
SSN
Address
Signature

Voice number (405) 954-4173.

I was never able to get the fax to work (over several days), and every time I called their voice number I got voice mail. I left several messages but never did get a return call.

I gave up and just mailed the letter in to:
Federal Aviation Administration
Aviation Data Systems Branch
PO Box 25082
Oklahoma City OK 73125

It takes four to six weeks to get the letter. Mine finally arrived yesterday (I requested it on Aug 30) much to my relief. It was the last thing I needed for my PACE application.

I sent the application in with a cover letter asking them to accept it without the FAA letter and got a interview for Nov 11.
