Are SIDs only established for airports with surface-based controlled airspace?
No.

SIDs tend to be established where there is enough traffic to warrant a charted clearance instead of ATC having to read a sometimes complicated clearance longhand. It's more based on the overall airspace than any individual airport. While it is certainly true that the presence of a busy airport is usually the impetus to have it, the SID will apply to most of the airports that are affected, whether controlled or not.

So for example, KFNL, a nontowered airport over 40 NM NE of KDEN, has with Class E beginning at 700 AGL has the same 5 SIDs as KDEN. (it even has a dedicated Clearance Delivery frequency)
