Previously reported Vision Problem

Here's the situation, you have a 1st class medical cert issued to you with the limitation, "Holder must wear corrective lenses" on your cert. Your vision is 20/40 (near-vision) correctable to 20/20 and 20/20 distant-vision. What happens if you go to the eye doctor for a routine checkup and receive a different prescription (stronger prescription = weaker eyes for argument sake) lenses. How do you go about putting this on the 8500 form?
Do you list it as a a change to one's medical problem?
Do you still put "previously report, no change" because it is so routine and therefore expected (assuming you still pass the vision tests fine)?

Now, instead of it being a simple near vision problem, what if is it something more abnormal, like Strabismus (lazy eye/cross eyed) but all it takes is a new lens prescription to fix the problem?

Strabismus is a condition where, for the original issuance of a 1st or 2nd class medical, an applicant needs to get an "Eye Evaluation Form" filled out by an eye specialist before a medical is issued. would this change how you would need to report said change in prescription?
