Pilot Services Agreement Contract

I am looking for a few 'Pilot Services Agreement' sample contracts, besides the standard one that is floating around the internet: http://www.jetpilotinc.com/policies.html

I'm trying to put together a contract that would be more of a long-term agreement, not just a one-day flying gig, but something that would also include aircraft management. If someone has another example or has more of an expanded contract, please send me a PM! Also, if you have any ideas of other things to include, I'd appreciate it.

The last gig I picked up (the current one), they had their own little format. I'll see if I can get a copy for you. It was basically an agreement saying I'm not an employee bla bla bla, fly for a year, bla bla bla...nothing like mine which is more focused on: Day rate, billing cycle, payment due, cancellation policy, etc.

If that's what you're looking for let me know...I'll be looking around my office to see if I can find it.

The contract/agreement I'm trying to put together would be for a somewhat long-term (not just a day or a week, I'm talking several months...) Pt. 91 flying job. So, it really isn't a contract flying job, but I would still like to establish some kind of agreement between me and the owner/client. I'd like to have different sections of the contract. So far I have Compensation & Benefits, Travel/Layover Expenses, Insurance Considerations, Termination of Employment and I'd like to add an Aircraft Training/Re-Current section. Also, I'd like to add some sort of Pilot's Code or Commitment section, that would have stuff in it like...

I will at all times observe the highest standards of my profession.

I will never knowingly jeopardize the safety of a flight by undertaking a risk to satisfy personal desires, nor will I fly when my mental or physical condition might lead to additional risks.

Etc. You get the idea.....

Any other ideas? Info? Good/Bad? Job would entail quite a bit of international flying/layovers, so is there anything I should add? Thanks.
