New medical since BP medication

Doc,

I will be going for my first medical since being placed on BP and cholesterol medication. Does the FAA have a form that I should give to my doctor to fill out before I go to my local AME? I have a doctor visit coming up and I'd like to bring the form with me if one exists or at least know what I should be asking my doc to do (if anything during that visit).

From reading the posts on high BP, I am not worried about being completely rejected for a medical, but I would like to make the process as expeditous as possible.

Thanks doc. If I am understanding the FAA info correctly, it sounds like I can just show up for the AME and that doc can do whatever is necessary (at his discretion) without me having to get or bring any info from my doc? Is my understanding correct?

Would I still have to go through the waiting period seeing that I've been on the same meds for about a year now?
 
No, take the information required from your doc to the AME, otherwise there will be a delay and some AME's just defer the medical (2 months with no medical:()
