Do I mail this form off to the national driving record place or can I go down to the DMV and have it printed off and notarized?
 
You have to mail the form off to the NDR office in Washington D.C. Remember if this is for an airline then request an airline search because they will only report the last 5 years, if you don't request an airline search your entire history will be reported. Driving suspensions, DUI's, and violent driving offenses are not reported by the S.O.S office.

Every NDR report request I've ever seen is mailed to the state agency.
