Has anyone ever filled one of these out? I may have inadvertently violated a FAR and was instructed filing one would be a good idea if I ever did. What all is involved and where can I pick one up?
The appropriate form is on there...just print it out and mail it in. The immunity policy should tell you a bunch too. Unless you did something criminal or crashed an airplane, this'll cover your ass.
I've filled out about four in my career thus far. They're free and may potentially come in handy when you least expect it.
I've had NASA ASRS forms on the website for the past two years: Go to the main page, look under "pilots toolbox" and select 'nasa asrs forms' or just click here.
You can actually fill it out online, click the 'print' button on your browser and it'll print a legible copy for ya!
I've been "lucky", everytime I've violated or was alledged to have violated an FAR ( twice that I can honestly remember ) I was operating in a Military status and was "immune" from the FAA ... albeit not from my command. I've only had one controller insist on proceding with the vilolation and I've never heard a thing from it. In the Military we don't use the NASA form, but I did give my command a heads up on what happened, they hate to be blind sided with stuff.

Starting this week. I'm ready to get in there, too. Four of us in "indoc" class, and two instructors are leaving JWN in the next two weeks. They're overwhelmed with stupids, so I'll either be loving or hating life in about two weeks...I'll let you know!!
