For reported criminal acts and aircraft accidents, NASA forwards the reports to the DOJ or NTSB respectively. For everything else, NASA de-identifies the reports, then releases it on their website. NASA does NOT disclose to the FAA. Unintentional violations of FAA regulations are not criminal acts (unless frequent, repeated, then they cross the line to intentional, and even then NASA doesn't send the info to the FAA, FAA finds out on its own).
Make sure you don't put anything in your "human factors" side of the report than can be traced back to you, and send it in.
