Letter to AME from optometrist format?

Hey Doc,

The local AME here in town where I am going to get my new medical (I used an out of town guy last time but I want to use the local guy just out of convenience and I know several people that use him), but there is a small issue.

I hear that his vision machine is pretty antiquated and is about from the 70s era. I have been told it doesn't work very well and it is just a real pain to use and actually might make it more difficult to see 20/20 than it should be.

I want to get my optometrist to just write my AME a letter saying I was tested by her and shown to have 20/20 vision and no colorblindness recently, but I don't know the format, and what all the letter from my Dr. to the AME should contain. She has never done a letter to an AME before since she has a relatively new practice, and I need to be able to tell her what all should be on there. If you wouldn't mind typing a little mock one out or something I would be very grateful.

Thanks Web, I guess I also have kind of a follow up question for the Doc. My last eye exam was in February. Would the FAA accept the information if it was dated on the form that far in the past? Or would they want a more recent examination to be done by my eye Dr.? If so, how recent should the eye exam information be from my eye dr to go onto the 8500-7 form.
