I have a question for you been there done that IFR guy's. On the Grand Canyon RNAV GPS 3 approach one of the IAF's (CRAMS) is also listed as an IF (intermediate fix). The link below will show you the approach but the (IF) has been removed. We are looking at an old plate but the question remains. Why is CRAMS a IAF and an IF.
 
It is usually because there is no intermediate segment on the approach procedure. When there is no IF formatted for the approach, the IAF usually signals the beginning of the intermediate segment. Often the intermediate segment begins at a point where you are procedure turn inbound to the final approach fix.

Remember the approach segments are - 1) feeder routes 2) initial approach segment 3) intermediate segment and 4) the missed approach segment.
