Diabetes type 2 medication combination

Hello,

I appreciate the help you can give me. I am an ATC and have my flight physical coming up. I have type-2 diabetes and am currently taking simvastatin, enalapril, glyburide, and metformin. My personal physician is suggesting I try Januvia due to spikes in my blood sugar. I have had bariatric surgery and anticipate lowering medication as my weight decreases.

My questions: Should I just stay with my current regimen of 4 medications? Can I add Januvia as a fifth medication and still pass my flight physical?

You cannot do it. Here is the FAA reference:

Januvia: may be acceptable under Special Consideration with an observation period of at least 14 days if the ATCS is not also taking a thiazolidinedione or metformin and at least 60 days if also taking a thiazolidinedione or metformin (if the ATCS has already been taking a thiazolidinedione or metformin for over 60 days and the Januvia is added in as a combined therapy then the 14-day waiting period applies). Januvia is not allowed with any other anti-diabetes medications.



You could do it with metformin or a glitazone (actos or avandia) but not in combination with glyburide.
 
Doc, I was diagnosed with type 2 diabetes on July 11. 2013 and started1000 mg metforman then. On August 9th my Dr. increased the metforman to 2000 mg/day and put me on 100 mg Januvia/day. Also on Aug.9th my 1Ac was down to 8.8. Can you tell me how long my wait period will be? Many thanks, Doc.
 
You must wait 60 days, then submit all records to the FAA for consideration for a Special Issuance medical certificate. You cannot fly until the FAA makes a decision.
