That's the name of Discovery's new show. It looks like it's going to be about NTSB investigation and analysis techniques. Has anyone else seen this? What do you think?

There's a number of high-profile accidents that have shaped flying.

EAL66 and DAL191 really brought out windshear awareness, for example.

AF90 refocused the importance of winter/deice procedures.

UA178 was a big focus on CRM

AA191 brought about attention on engine out procedures on T/O

TWA843 refocus on RTO procedures

NWA255 and DAL1191 refocus on T/O checklist procedures and configuration importance.

Among others....
 
Hey, sorry Mike. I didn't mean to imply that the name was a bad thing. I just think it's a really interesting topic for a show and I can't wait to see how it turns out. I agree with all the accidents that have shaped what flying is today.

Oh no partner, no implication taken! It's all good! I was just building on your initial post with some info on crashes, off the top of my head, that have changed aviation in a major way. Your initial post was a great lead-in to my comments! :)

UA232 for multiple reasons (CRM, maintaining control with zero hydraulics/disfigured tail section/both ailerons fixed in down position/alternating engine thrust/impact of phugoid etc)
 
Hi! This is safety engineer! Please don't speculate on the cause of air crashes. They are horrible tragedies and it si important to let the NTSB complete their investigation without interference! Even if the investigation is complete! Please say a prayer for the victims and thank the first responders!

One important safety feature is to never have a Flight 191.

DAL 191
Comair 191

I would also look at the mid-air in the 50's between UAL and TWA over the Grand Canyon. That wreck had huge implications for the future of air travel.
 
UA232 for multiple reasons (CRM, maintaining control with zero hydraulics/disfigured tail section/both ailerons fixed in down position/alternating engine thrust/impact of phugoid etc)

And maybe it's not a good idea to have the hydraulic lines from 3 independant systems running right next to each other... near an engine...

That is what you call, BAD LUCK! I couldn't image having that happen to me. I wouldn't know what to think....oh cr@p, maybe that's the best statement.

Ha ha... Thats the exact one that popped in my head as soon as I read this thread. The TWA bird was a Connie but I don't remember what the UAL plane was and don't really have time to look it up.

I agree fully. I'm far from superstitious, but I will try to NEVER fly on a flight 191.
