Corneal Abrasion and ATC

I'm heading to the OKC PEPC later this week, and was wondering if a corneal abrasion, which occurred 2 months ago and has since healed, is going to disqualify me. Please excuse this brief moment of paranoia anic:

Also, I am assuming I must list my related ER and subsequent opthomologist visits on my 8500-8... am I correct in this assumption?
